These are just a very few of Social
Security's own forms, and were the most current version when they were posted
here. To be sure you are working with the most current version we recommend that
you go to the Social Security Administration's website, www.ssa.gov,
and download the version that is available today.
Work history report (Why
the work history report form? Because you must inform the Administration if you
are getting disability benefits but you are trying to go back to work. If you
are successful in going back to work, there are programs to help you make the
transition off of benefits and on to a salary. But if you keep accepting
disability benefits checks at the same time that you are working and earning a
paycheck, Social Security will eventually figure it out and come after you. They
will want back the money that you have been overpaid, and by the time they
notice, the total amount could be thousands of dollars. Don't risk it.
When you return to work, notify Social Security in writing -- and keep a copy
for your own records! This form is great for giving Social Security this sort of
an update.)
Medical Report on Adult with HIV,
form SSA 4814. This tough-to-find form
is reproduced here in PDF format. This is the 2000 version with 2002
updates, and we will post a
new version as soon as it is released by the Social Security Administration and
someone lets us know. Note that this form is not available on the
Administration's website, or in most field offices.

the public domain. They are not copyrighted, so download, print, copy, share and
